Quick fix: It does not remember your last chat
Instead of Continue with the approach we agreed on yesterday.
Try Yesterday we settled on [the approach, one sentence]. Continuing from there: [today's ask].
Restate the conclusion, not the whole conversation.
Why this works
By default, each conversation starts blank; yesterday's chat is not there. The trap is that the model will not say so. Asked to "continue as discussed", it infers a plausible discussion and continues that, confidently. You then build on an agreement that never happened. Where a memory feature is on, it holds durable facts, not transcripts, so the specific decision you are pointing at still may not be in it. One restated sentence removes the guesswork either way.
When this applies
You typed "as discussed", "like last time", or "you remember" — and the thing you mean lives in another conversation.
